App successful installation page keeps appearing

Ok so everytime i open chrome i get the installation page, so i think really i installed weeks ago and this comes up everytime i close chrome down (no matter how i close chrome)... PLEASE HELP!!!

Reset chrome, reinstall your extensions or try first a new fresh profile (and again ..reinstall all your addons)
