How can I block Forsee surveys?

Many sites like Edmunds.com have a popovers that often ask me to take a survey.
I'd like to block these Forsee survey popuovers.

However it seems that the latest version of Adblock plus has a different interface.
I'm trying to figure out how to "block an ad by it's URL", in the latest version of Adblock Plus for Firefox, as instructed in the article.
